Thyroid antibodies have been measured in suspected cases of thyroid disease since 1956 when Roitt first demonstrated their presence in patients with Hashimoto's thyroiditis.
Thyroid antibodies appear to play an important role in disease pathogenesis.
Antibodies to Tg and TPO vary independently of one another. Circulating autoantibodies to TPO are present in the serum of up to 90% of patients with autoimmune thyroid diseases. In particular, anti-TPO is associated with most cases of Hashimoto's thyroiditis, atropic thyroiditis and postpartum thyroiditis, and is often present in Graves' disease. Tg antibodies are associated with the same spectrum of thyroid diseases as TPO, although anti-Tg is less prevalent than anti-TPO (as low as 32%).
The presence of anti-Tg alone is relatively uncommon. However, Tg antibody testing is particularly important in patients with thyroid carcinoma, in which serum thyroglobulin measurements are used as a tumour marker. In such cases Tg antibodies must also be measured, since their presence may interfere with the serum Tg measurement.
